Elevate Her Wellness Retreat Waiver of Liability

Participant Agreement and Acknowledgment of Risk

By signing this waiver, I acknowledge that I have voluntarily elected to participate in the Elevate Her Wellness Retreat, and I understand and accept the following terms and conditions:

  1. Acknowledgment of Risks:
    • I am aware that participating in physical workouts carries inherent risks, including but not limited to, physical injury, muscle strain, sprains, and other potential health risks including death.
    • I confirm that I have consulted with my physician and have received clearance to participate in physical workouts and activities at the retreat. I voluntarily assume all risks associated with participating in these activities.
  2. Inner Healing Sessions:
    • I understand that inner healing sessions at the retreat will be conducted by an Issue Focused Minister from Restoring the Foundations.
    • I acknowledge that I will be seeing a Minister who will listen, support, encourage, pray, and minister to me to help me overcome my problems and grow in my Christian life. I accept that the Minister may not be a licensed counselor.
  3. Assumption of Risk and Release of Liability:
    • I assume all risks related to attending and participating in the Elevate Her Wellness Retreat.
    • I agree not to hold the hosts of the retreat, their families, businesses, or heirs liable for any injuries, damages, or losses that may occur as a result of my participation in the retreat.
  4. Additional Terms:
    • I acknowledge that the retreat may include a variety of activities, including physical workouts, inner healing sessions, and other wellness activities.
    • I understand that participation in the retreat is entirely voluntary, and I have the right to decline participation in any activity that I am uncomfortable with.
    • I grant permission for Elev8 Her to use photographs and visual images of me taken at the retreat for promotional marketing, and informational purposes; I understand that no audio recordings will be made, and I waive any right to compensation or approval of such use.

By signing this waiver, I affirm that I have read and understood the contents of this document and that I voluntarily agree to the terms and conditions outlined above.
